Please complete the following form to finalize any specific choices or "TBD" items within (3) weeks
from date on contract. Additional charges or credits may be due upon examination of your requests. Once
all of your final selections have been made, a more exact installation date may be issued along with a
revised consolidated contract indicating all new information based on your final choices. Please allow 10
days to process and confirm your requests. Feel free to consult your designer for more specific questions.
Change orders after the (3) week window may result in a 30% up charge depending on specifics and
logistics associated with your request.
